When you mention that your device is always in extended sleep mode, i suppose that you mean that it is in permanent sleep, no advertising and it is not connected, there is no way to do this, if the device is sleeping constantly with no TX RX activity, there is no way to receive the signal over BLE to wake up. You can wake up the device only via an external wake up or a ke_timer.
